An appointment is required to see the doctor. Appointments are available from 8:30 am to 4:30 pm. If you have an emergency, you can be accommodated the same day. However, a wait may be encountered.
During your first visit the front desk coordinator will give you introductory forms to fill out. We are required to obtain necessary personal information, insurance information and emergency contact information. Please bring a picture ID and your insurance card. We also ask that you bring a list of your medications with you each visit. Once these forms have been filled out, a medical chart can be prepared and you will be entered into our computer system.
Cardiology Associates of Boca Raton and their staff will make every effort possible to see our patients on time. Unfortunately, emergencies do arise and delays may occur.
Note: Please notify the front desk if you changed your address, telephone or your medical insurance.
Referral and Scheduling
The referral coordinator will coordinate referrals and diagnostic testing appointments. Due to the constraints of managed care and HMO’s it often takes several days to get approval for a test.
